Copies
of fire reports are available by contacting the Northlake Fire
Protection District, Monday through Friday 9:00am to 4:00pm at (708) 562-3182.
Victims Only of a fire incident, legal entities, and insurance companies may request
fire reports. There is a $20. charge for copies. This charge is waived for
victims of the fire.
In
order to obtain a copy of an ambulance report, an "Authorization to
Release Medical Information" form signed by the patient or other legally
authorized individual must be submitted. The patient can directly request an
ambulance report. Copies of ambulance reports may be obtained at the Northlake
Fire Protection District Administration office, Monday through Friday 9:00am to
4:00pm. For more information call (708) 562-3182. There is a $20. charge for
copies. This fee is waived to residents of the Northlake Fire Protection
District.
