Birth and Death Certificates
Birth and Death Certificates must be obtained from the County Clerk's Office in the county in which the event occurred. If the birth/death occurred in Cook County, you may obtain a copy of the birth or death certificates from the locations listed below.

Marriage Licenses
Northlake City Hall does not issue marriage licenses. Marriage licenses must be obtainded from the Cook County Clerk's office at the following locations:

Vehicle Stickers
Residential Truck and Vehicle Stickers may be obtained at Northlake City at no cost to residents. Vehicle Stickers are valid for four years and Truck Stickers are valid for one year. In order to obtain a sticker, you must have a valid registration card. Non-commercial trucks and vans are issued regular four-year car stickers. Commercial trucks require a new sticker each year.

Pet Licenses
Pet Licenses are available for a two dollar ($2.00) fee. The applicant must provide proof of Rabies inoculations. License period is for one (1) year. This form is available to download in .pdf format.

The City also requires the registration of any dog that falls under the classification of "vicious" or "dangerous." This form is available to download in .pdf format.

Solicitation Licenses
Solicitors are required to apply for a Solicitation License at least fifteen (15) days prior to the date in which the license shall be effective. Applicants must provide proof of Liability Insurance. The License period may be approved for 30 days. This form is available to download in .pdf format.

Fishing and Hunting Licenses
Fishing and Hunting Licenses, Permits and Stamps may be obtained at Northlake City Hall. Fees vary. Call (708) 343-8700 for more information on fees.

Tickets
Payment of Traffic and Code Violations Tickets may be made at City Hall or through the mail. If you want to dispute the ticket you may attend the administration hearing dated noted on your ticket.

Freedom of Information Requests
When requesting copies of City Records, the City of Northlake requires that you fill out the appropriate request form. Pursuant to the FOI Act, you must receive a answer to your request within 7 business days. The fee for FOI Requests varies depending on the number of pages. Call (708) 343-8700 for more information.
